These Terms of Service ("Terms") govern your access to and use of the website https://www.jackzamoramd.com (the "Site"), which is operated by Ophthalmic Consultants of Colorado Prof LLC, doing business as Jack Zamora MD Cosmetic Surgery and Aesthetics ("we," "us," or "the practice"). By accessing or using the Site, submitting a form, or communicating with the practice through the Site, you agree to these Terms. If you do not agree, please do not use the Site.

No Medical Advice; No Doctor-Patient Relationship

The content on this Site, including descriptions of procedures, treatments, before-and-after photographs, blog posts, and videos, is provided for general informational purposes only. Using this Site, submitting an inquiry, or requesting an appointment does not create a doctor-patient relationship. A doctor-patient relationship is established only through an in-person or formally conducted consultation with one of our providers. Individual results vary, and no specific result is promised or guaranteed for any procedure or treatment. Candidacy for any procedure is determined only through consultation with a provider.

Appointments, Fees, and Cancellations

Appointment requests submitted through the Site are requests only and are not confirmed until you receive confirmation from the practice. A consultation fee is required when booking with our surgeons; the fee is collected at the time of scheduling, holds your appointment, and is applied as a credit on your account toward treatment or surgery performed in our office. A credit card on file is required when booking med spa services. We require at least 24 hours notice for appointment changes or cancellations. Appointments cancelled or changed with less than 24 hours notice, and missed appointments, may result in a charge to the card on file in the amount of the cancellation fee disclosed at the time of booking. Fees and policies are stated during scheduling and are subject to change.

SMS Terms and Conditions

By providing your mobile phone number to Jack Zamora MD Cosmetic Surgery and Aesthetics, whether through a form on this Site, verbally during a call with our staff or scheduling assistant, or in person at our office, you consent to receive text messages from the practice regarding appointment scheduling, confirmations, reminders, rescheduling, follow-up care, pre- and post-treatment instructions, account notifications, and replies to your inquiries.

  • Message frequency varies based on your appointments and interactions with the practice.

  • Message and data rates may apply according to your mobile carrier plan.

  • Opt out at any time by replying STOP to any message. You will receive a single confirmation of your opt-out, after which no further messages will be sent unless you opt back in.
