SMS Terms and Conditions

Sender and Program

Jack Zamora MD Cosmetic Surgery and Aesthetics sends text messages to patients and prospective patients who directly and voluntarily opt in to receive messages from the practice.

The messages may concern inquiries, appointment requests, appointment confirmations, appointment reminders, appointment changes, and related non-clinical administrative follow-up.

This campaign is operated by Jack Zamora MD Cosmetic Surgery and Aesthetics. It does not send messages on behalf of another business, affiliate, advertiser, lead generator, or unrelated sender.

SMS Consent

Recipients opt in through the public Jack Zamora MD Cosmetic Surgery and Aesthetics contact or consultation form at https://www.jackzamoramd.com/contact.

The form displays a separate SMS consent checkbox that is optional and unchecked by default. The checkbox identifies Jack Zamora MD Cosmetic Surgery and Aesthetics as the sender and explains the types of text messages that may be sent, message frequency, message and data rates, STOP instructions, HELP instructions, and that consent is not a condition of purchase, treatment, or receiving care.

The contact or consultation form can be submitted without checking the SMS consent checkbox.

Entering a mobile telephone number alone does not constitute SMS consent. Accepting these Terms of Service does not constitute SMS consent. Accepting the Privacy Policy does not constitute SMS consent. Consent must be affirmatively provided through the separate SMS consent checkbox.

This campaign does not use verbal opt-in, in-person opt-in, Booker opt-in, third-party opt-in, or text-message keyword opt-in.

Messages, Frequency, and Cost

Messages may concern inquiries, appointment requests, appointment confirmations, appointment reminders, appointment changes, and related non-clinical administrative follow-up.

Message frequency varies based on the recipient's inquiries, requests, appointments, and interactions with the practice. Message and data rates may apply according to the recipient's mobile carrier plan.

Opt-Out

Recipients may opt out of text messages at any time by replying STOP to any message from Jack Zamora MD Cosmetic Surgery and Aesthetics.

After receiving an opt-out request, the practice may send one confirmation message. No additional text messages will be sent unless the recipient separately opts in again.

No Condition of Purchase or Treatment

Consent to receive text messages is not a condition of purchase, treatment, receiving care, making an appointment, submitting an inquiry, or using the website.

Mobile Information and Consent Sharing

Jack Zamora MD Cosmetic Surgery and Aesthetics does not sell, rent, share, transfer, or provide mobile telephone numbers, SMS opt-in records, or messaging consent data to third parties or affiliates for their own marketing or promotional purposes.

The practice does not provide SMS consent or mobile information to lead generators, data brokers, affiliate marketers, advertisers, or other organizations for marketing or promotional purposes. SMS consent is specific to Jack Zamora MD Cosmetic Surgery and Aesthetics and is not transferable to another business, affiliate, advertiser, lead generator, or unrelated sender.

The practice may use messaging vendors and service providers solely as necessary to operate and deliver text messages on behalf of Jack Zamora MD Cosmetic Surgery and Aesthetics. Those vendors and service providers may process mobile information and consent records only to provide services to the practice. They may not use that information for their own marketing or promotional purposes and may not sell, rent, transfer, or provide it to other parties for those purposes.

A mobile telephone number provided to a third-party booking or scheduling platform does not, by itself, constitute consent to receive text messages from Jack Zamora MD Cosmetic Surgery and Aesthetics. A third-party platform is not an opt-in method for this campaign.

Text Message Limitations

Text messages may not be encrypted. Do not send sensitive medical information, emergency information, or urgent medical information by text message.

Text messaging is not intended for emergencies or urgent medical concerns. Call 911 for an emergency or contact the practice directly.

Wireless carriers are not liable for delayed or undelivered messages.
